When a player is injured at a game or practice or when a player or AYSO volunteer has tested positive for COVID-19 or had close contact with someone who tested positive for COVID-19 - must be sent to Safety Director ([email protected])
Participation Return to Play Release form
when for a concussion this is to be signed by your doctor before a player is cleared to play - must be sent to Safety Director ([email protected]). When for COVID-19 it must be signed by a parent or guardian.

Player Practice Release Form
This general release form is to be signed by a parent when requesting a player to leave with a non-parent or leave practice on their own (given to coach).
SOCCER ACCIDENT MEDICAL INSURANCE
AYSO purchases Soccer Accident Insurance (“SAI”), which pays excess medical costs up to $50,000 maximum per accident to an insured person for accidental bodily injuries incurred as a direct result of participation in a covered activity subject to the policy terms, conditions and exclusions. Eligibility for benefits is 52 weeks from the policy effective date or the date of a covered accident. Download the SAI Brochure below for more detailed explanation of the plan.
